Bilingual Case Manager- Spanish & Portuguese Speaking

We are currently seeking bilingual Case Managers- Spanish and Portuguese speaking for Elders Services Division in our Providence office. These Case Managers will have the overall clinical responsibility for assessing older persons, developing plans of care, arranging for and monitoring services. Assessment goals are to identify needs, make referrals to appropriate service systems, and follow-up.  Position will help improve the quality of life for those elders and promote independent living by providing counseling, advocacy, education and support systems to the elderly living in the community.

In addition to:

  • Promote independent living by providing counseling, advocacy, education and support systems to the elderly living in the community.
  • Maintain a working knowledge of community resources and a working knowledge of payer requirements.
  • Assist in telephone screening of elders and in-home assessment of Senior's needs.
  • Develop plans of care to address needs and problems identified, and to incorporate services needed to enhance current support system.
  • Evaluate the client’s response to the plan of care and update the plan based on client’s response.
  • Provide support to client regarding daily living issues; conduct home visits as necessary.
  • Respond to Protective Service Cases.

Requirements:

  • Bachelor Degree in Gerontology, Social Work, or related field with Elder experience.
  • Valid driver’s license required.
  • Bilingual, Spanish or Portuguese required
  • Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
